Dear Patients and Friends,
It is with much regret that we must inform you that Great West Family Care, PC is ceasing operations effective April 20, 2012. We will not be available to provide you with medical care after that date. If you need to get in to the office to see Dr. Hoffman or Dr. Carroll before April 20th, please call 360-574-9730 to schedule an appointment. This notification also applies to patients participating with New West Medical Care.
Finding a New Healthcare Provider
It is important for you to locate a new physician to assume your care. We encourage you to do so at your earliest opportunity to ensure a smooth transition to your new provider. For assistance in locating a new physician you should contact your medical insurance plan for participating providers or contact the Clark County Medical Society or other physician referral service.
Dr. Hoffman will be joining Kaiser Permanente in mid-May. (Seeing a Kaiser Permanente provider requires enrollment in a Kaiser health plan.) If you wish to continue with Dr. Carroll, he will be joining Ridgefield Family Medicine in mid-May.
Obtaining a Copy of Your Medical Record
Upon request, we will make a copy of your medical record available to you or a physician you designate. To comply with state and federal law, we require a written authorization to release your records. We have sent a copy of this letter and an authorization form to each of our patients. You can also download the form by clicking here.
There is no cost to have records sent directly to your new medical provider. To obtain a personal copy, there is a small fee to cover office costs and postage. Please contact us for more information on fees for personal copies.
To request your records for the last two years, please complete and sign the authorization form, and return it to the following location. If you need records from before 2010, please call and leave a message at 360-574-9730.

Dr. Hoffman will be available by phone (360-574-9730) for any inquiries or requests between April 21st and April 30th, before starting her work at Kaiser Permanente. Dr. Richard Carroll will continue practicing in Clark County.
